This is an example to show how a summary can be shaped. It is not generated from your answers. To create your own summary from what you have written, use the birth timeline.
What I remember
My concerns started the evening before I went in. I noticed my baby was moving differently and I felt something was not right. I rang triage and was told to come in for monitoring.
What I was told
- The trace looked reassuring at the time.
- I was told to go home and come back if anything changed.
- Later, my labour was described as “precipitous” but no one explained what that meant for us.
What I still do not understand
- Why my earlier concerns were not documented.
- Why the plan changed without anyone explaining it.
- Who was the senior clinician responsible for our care.
Questions I want answered
- Can I have a copy of my notes?
- Were my concerns recorded the first time I raised them?
- Could anything have been done differently?
- Can I be referred for birth reflections and mental health support?
