What to write down

  • · What concern was raised
  • · What you were told
  • · What the plan was
  • · When the next review should happen
  • · Who is responsible for the next decision
  • · Any changes in symptoms, movements, waters, bleeding, contractions or pain
  • · Questions still unanswered

Ask for it to be documented

“Please can you document that I raised this concern, what I reported, what was checked and what advice was given?”

Ask them to document

  • · “Please can you document the plan and review time?”

You can ask for your concern and the advice given to be recorded in your notes at any point in your care.
